Patient Accounts

• The Authorization for Release of Semen form needs to be completed and signed by the physician.
• The Patient Registration form and the Patient Consent for Therapeutic Donor Insemination form need to be completed by the patient.
• If married, the patient’s spouse is also required to sign the patient forms.

For patients who prefer HOME INSEMINATION, the following forms must be completed and returned to MSB by either fax or by mail.

• Patient Registration Form
• Patient Consent for Therapeutic Donor Insemination Form
• Authorization for Purchase of Semen Form
• Authorization and Release for Home Insemination Form

Accounts are set up the same day the signed documents are received.
