In order to purchase donor specimens from Midwest Sperm Bank, a cryobank account with MSB is required. MSB offers two types of cryobank accounts depending on the preference:

• Physician Accounts
• Patient Accounts

Orders may be placed by you or by your physician's office by:

• Calling 630-810-0217, Monday through Friday from 8:30 a.m. to 3:30 p.m. Central Time.

Note: If the account has been established in your physician’s name, only your physician’s office may place the order. If the account has been established in your name, then either you or your physician’s office may place the order.

Patients with their own accounts can pay either by money order, cashier’s check or by credit card. We accept Visa, MasterCard, American Express, or Discover Card.

Payment by credit card, which is more convenient and efficient, can be done at the time of placing an order. All patient pre-pay orders must be authorized by a licensed physician or health care provider. A credit card company payment denial may result in cancellation of the order.

Orders for cryopreserved donor sperm can be placed at any time during the month. Orders are taken and shipped on a first request basis. You must place your order at least three (3) business days prior to the date needed in order to ensure prompt and economical delivery of the requested donor specimens.

Note: Same day orders must be placed by 12 p.m. for orders being shipped, and by 2 p.m. for pick up orders. Same day orders have an extra fee of $175.

Midwest Sperm Bank will deliver to a physician's office/hospital as well as a patient's home address.
